How do you apply for a cannabis prescription in NZ?

New Zealand blog legalise.org.nz recently posted a link to an application form which they suggest can be used by a doctor to apply for approval to prescribe cannabis in NZ.
The form appears to be extremely well formatted and specifically requests "non-pharmaceutical" medical cannabis. It includes the formal application form found on the Ministry of Health website and is based off the successful application letter from the CCDHB to the Ministry of Health in respect of "Alex Renton".

We do not know any more details on this particular form or if it has been successfully used by anyone in New Zealand apart from the precedent case, please let us know if you know of anyone who's attempted such an application!
